Agreement to Terms
By engaging Injury Care Coordination LLC for case management services, you agree to be bound by these Terms and Conditions. Please read them carefully before proceeding with our services.
Services Provided
Injury Care Coordination LLC provides professional medical case management and care coordination services. Our services are designed to facilitate communication between healthcare providers, patients, and payers, but do not constitute medical advice or treatment. We do not provide direct medical care or make medical decisions on behalf of patients.
Client Responsibilities
Clients agree to:
- Provide accurate and complete information necessary for case management
- Respond promptly to requests for information or documentation
- Maintain open communication with our case management team
- Follow through with recommended medical appointments and treatments
- Notify us of any changes in health status or contact information
Confidentiality
We maintain strict confidentiality of all client information in accordance with HIPAA and other applicable privacy laws. Information will only be shared with authorized parties as necessary for care coordination or as required by law.
Fees and Payment
Service fees are determined based on the scope and complexity of case management required. Payment terms will be outlined in individual service agreements. We accept payment from insurance companies, employers, legal representatives, or individual clients as appropriate.
Limitation of Liability
Injury Care Coordination LLC provides case management services in good faith and with professional competence. However, we are not liable for medical outcomes, treatment decisions made by healthcare providers, or actions taken by third parties. Our liability is limited to the fees paid for our services.
Termination of Services
Either party may terminate case management services with written notice. Upon termination, we will provide a summary of services rendered and transfer relevant documentation as requested. Fees for services provided up to the termination date remain due and payable.
Professional Standards
Our case managers adhere to professional standards established by the Commission for Case Manager Certification (CCMC) and other relevant professional organizations. We maintain appropriate professional liability insurance and comply with all applicable state and federal regulations.
Dispute Resolution
Any disputes arising from these terms or our services will be resolved through good faith negotiation. If negotiation is unsuccessful, disputes will be subject to binding arbitration in accordance with Michigan law.
Changes to Terms
We reserve the right to modify these Terms and Conditions at any time. Changes will be effective upon posting to our website. Continued use of our services after changes constitutes acceptance of the modified terms.
